Always round DOWN to the previous whole number if the calculated DOS contains a decimal.

DOS = Total Doses in Container ÷ Daily Dose
Example 1: ProAir Inhaler (200 puffs), "Inhale 2 puffs by mouth twice daily as needed"
2 puffs × 2 times/day = 4 puffs/day
200 puffs ÷ 4 puffs/day = 50 DOS
Example 2: Fluticasone Nasal Spray (120 doses), "Spray once in each nostril twice daily"
(1 spray × 2 nostrils) × 2 times/day = 4 sprays/day
120 doses ÷ 4 sprays/day = 30 DOS

DOS = (mL Dispensed × 20 drops/mL) ÷ Drops Used Daily
Note: Standard conversion is 20 drops per mL for most solutions and suspensions. Some medications have different drop conversions - IC+ will automatically calculate these.

Example: Humulin 70-30 insulin (10 mL), "Inject 20 units subcutaneously three times daily"
Dispense #2 vials (20 mL total)
20 mL × 100 units/mL = 2000 units
20 units × 3 times/day = 60 units/day
2000 units ÷ 60 units/day = 33.3 → 33 DOS
Important: Always follow manufacturer in-use (opened) guidelines. Example: Lantus vials have a 28-day in-use limit, even if calculated DOS is higher.

Calculation Steps

  1. Calculate Total Units per Day:
    (Prescribed units per dose + Priming units) × Doses per day = Total units/day
  2. Calculate Days per Pen:
    Units per pen ÷ Total units/day = Days per pen
  3. Calculate Pens to Dispense:
    Days prescribed ÷ Days per pen = Number of pens
  4. Calculate DOS:
    (Pens dispensed × Units per pen) ÷ Total units/day = DOS
Critical: Insulin pens require priming before each injection. Standard priming is 2 units per dose. This waste must be included in DOS calculations.
Example: Novolog FlexPen (15 mL box = 5 pens × 3 mL = 1500 units)
"Inject 20 units subcutaneously three times daily with meals"

Step 1: Total units/day
(20 units + 2 priming) × 3 = 66 units/day

Step 2: Days per pen
300 units/pen ÷ 66 units/day = 4.5 days/pen

Step 3: DOS for 1 box
4.5 days/pen × 5 pens = 22.5 → 22 DOS
In-Use Guidelines: Follow manufacturer guidelines (typically 28 days max after opening). If calculated DOS exceeds in-use limit, annotate prescription and add "discard after X days" to directions.

Multi-Use Pens (Ozempic)

Calculate based on mL per dose and total mL in pen.

Maintenance Dose Example: Ozempic 2mg/3mL pen
"Inject 0.5mg subcutaneously once a week"
0.5mg = 0.75mL per dose
3mL ÷ 0.75mL = 4 doses
4 doses × 7 days = 28 DOS
Initiation vs. Maintenance: If prescription has both initiation/starter dose and maintenance dose, it CANNOT be refilled. Contact prescriber for separate prescriptions.

General Formula

Step 1: (mLs per injection × Frequency per week) = Total mL/week
Step 2: (Total mL in vial ÷ Total mL/week) × 7 days = DOS
28-Day Rule: If MDV DOS exceeds 28 days, switch to single-dose vials (SDV) if available. If SDV not available, annotate prescription and add "Discard Remainder after 28 days" to directions.
Testosterone Example: Testosterone Cypionate 200mg/mL, 10 mL vial
"Inject 0.5mL IM twice weekly"

Step 1: mL per week
0.5mL × 2 = 1mL/week

Step 2: Calculate DOS
(10mL ÷ 1mL/week) × 7 days = 70 DOS

Problem: Exceeds 28-day limit!
Solution: Switch to 1mL SDV → Dispense 8mL for 28 DOS

Special Rule: If prescriber indicates duration (e.g., "for 10 days") and not all quantity will be used, add "Discard Remainder" to directions and use prescriber's indicated duration as DOS.

100mL ÷ 10mL/day = 10 DOS
